Paperback book glued together on the spine. The covers are grey and have discoloration along the edges. The title and an image of boats at dock are on the front cover. There is a yellow square with a circular cut-out for the picture of the boats. Surrounding the circle are 10 ovals with city names and their codes inside. On the back cover, the Pacific Telephone and Telegraph Company's address is in the top left corner. The Postage Paid square is in the top right corner. There is also a silhouette of an evergreen tree with the words, "Keep Oregon Green" inside it. A 0.5" yellow band goes around the bottom of both covers. On each cover, inside the yellow line, it says, "See The Classified In This Directory; Its Yellow Pages Tell You Where To Buy It." In the center of the yellow band is an illustration of an open book that has the words "Yellow Pages" inside it. There is also a hole punched through the upper left corner. Size: 6" x 9"
